Welcome to the Anthem resale and lending information system. The form below may be completed and up to three (3) documents uploaded in order to request the documents listed. The intention is to complete requests within five (5) business days. All documents will be delivered via email attachment(s) in PDF format. To mail a request, please send it along with payment to:

Resale/Disclosure Package [$400]
For documents used by title companies for the resale of real property.
Payment through the title company is expected concurrent with COE. Note that the Owners Account Balance Statement is only valid for 15 days from date of issue. This package includes providing an updated Balance Statement prior to COE (upon request). Please note: If the updated balance statement is requested within two (2) business days of COE, a $100 rush fee will apply. Additionally, if an update request is made 30 days after the last requested package, a $50 update fee will apply. 

Refinance Statement [$75]
For documents used by title companies, mortgage companies, lenders and attorneys when real property is being refinanced.
Note that the Owners Account Balance Statement is only valid for 15 days from date of issue, so please order this no more than 15 days from expected date of loan closing. Payment will be charged to the homeowner's account.

Lender/Mortgage Package [$100]
For documents used by lenders and/or mortgage companies as part of a mortgage application process.
Includes a Uniform PUD/Condo Questionnaire, Owner Account Statement, current Financial Statements and current Budget. Note that the Owners Account Balance Statement is only valid for 15 days from date of issue, so please order this no more than 15 days from expected date of loan closing. PLEASE NOTE: the lender must remit a company check right away (no exemptions). We do not accept personal checks from future homeowners, and we do not wrap this fee into any closings.

Governing Docs Package [$50]
For governance documents of the Master Entity and the HOA.
Includes Articles of Incorporation, Bylaws, CC&Rs, Supplementals, Board Resolutions and other community governance documents. PLEASE NOTE: the lender must remit a company check right away (no exemptions). We do not accept personal checks from future homeowners, and we do not wrap this fee into any closings.

Uniform PUD/Condo Questionnaire (only) [$75]
Documents used by Lenders, Title Companies, Realtors and other parties.
Includes a Uniform PUD/Condo Questionnaire. PLEASE NOTE: the lender must remit a company check right away (no exemptions). We do not accept personal checks from future homeowners, and we do not wrap this fee into any closings.

  • NO refunds will be allowed after the documents are delivered.
  • Real property resales involves TWO (2) entities, each of which is obligated to provide documents. Each residential property belongs to one of three HOAs (Anthem Parkside Community Association, Anthem Country Club Community Association  or Village at Anthem Condominium Council of Co-Owners) and also is governed by the master entity, the Anthem Community Council. When documents are ordered, the order will provide documents for both the HOA the property resides within AND the master entity.
